An unexpected fall or other injury in a store, office, apartment building, or public space can be disorienting. If you are able, preserving basic information about the location and circumstances may help you explain what occurred later.

Note the location and conditions

Write down the exact area where the incident happened and what you observed nearby, such as a spill, uneven surface, obstruction, or lighting condition. If it is safe and appropriate, take photos from several angles and include a wider view that shows where the condition was located. Do not put yourself or anyone else at risk to collect evidence.

Identify reports and witnesses

Ask whether an incident report was created and how to obtain a copy. Note the names or contact details of witnesses and any staff members you spoke with. Keep your own factual account, including when you noticed the condition and what happened immediately afterward.

Save records and get appropriate care

Seek medical attention when needed and keep related records and receipts. Save messages, photographs, and any correspondence about the incident in their original form. Avoid guessing about causes or making assumptions when documenting what you personally observed.
